Breast Lift Surgery (Mastopexy)
Breast lift surgery is a surgery to lift the breasts and alleviate extreme sagging, uneven breasts, breast deformity, or a wide cleavage gap, which can be caused by aging, breast size, genetics, weight loss, or breastfeeding. Excess breast tissue and skin are adjusted, and excess fatty layers are removed. Moreover, surgeons can reposition and lift the nipples or reduce the large cleavage size during this surgery.
If patients’s areolas are too large, surgeons will reduce the areolas to fit the proportion of the breasts. Breast reduction surgery cannot result in fullness of the upper breasts or cleavage in every case; thus, patients might have to undergo breast lift surgery simultaneously. In some cases, if patients’ breasts are too large, they might have to undergo breast reduction surgery in conjunction with breast lift surgery.
What is the level of your breast sagging?
- Normal Level: In a normal condition, the position of the nipple is centered on the breast, pointing slightly forward, and there may be no need for lifting.
- Slight Sagging: The nipple is positioned slightly lower, pointing somewhat downward, and it may not necessarily require lifting. It varies based on individual preferences.
- Moderate Sagging: There is noticeable sagging with the nipple positioned lower, and it requires clear lifting.
- Severe Sagging: The breasts sag significantly, almost reaching the waist. The nipple and areola point downward, nearly touching the ground.
Potential Causes of Sagging
- Pregnancy – During breastfeeding, hormones cause the breasts to stretch. After weaning, the breasts stop producing breast milk and become smaller.
- Aging – As we get older, muscles and skin gradually lose their elasticity.
- Gaining weight – Fats are stored in breast tissue, causing breasts to become larger, which can result in saggy breasts.
- Fast weight loss – Losing weight the wrong way causes the breasts to shrink very quickly, and the skin loses its firmness, which leads to excess skin.
- Genetics – Breast size is mostly determined by genetics, which can cause them to sag easily.
- Exercise – Playing sports or exercising without wearing supportive clothing can cause breasts to sag easily.
Determining the Surgery Preparation
Breast lift surgery is determined on a case-by-case basis, and the surgical approach used is determined by the following characteristics:
- Breast size and shape
- Areola sizes and positions
- Breast elasticity
- Excessiveness of skin and elasticity

Breast Lift Techniques As follows:
1. Crescent Breast Lift
This surgical approach is suited for patients with minor sagging or nipples that rest 1 centimeter lower than the average. Surgeons make crescent-shaped incisions around the areolas to slightly lift both areolas and nipples. This surgical approach can lift areolas up to 2-3 centimeters and 1-3 centimeters for nipples.
2. Donut Breast Lift
This surgical approach is suited for patients with moderate sagging or nipples that rest more than 1 centimeter lower than the average. Surgeons make donut-shaped incisions around the areolas and pull the skin to make them tighter and more firm. The scarring from this surgical approach looks more natural than others.
3. Lollipop Breast Lift
This surgical approach is suited for patients with intense sagging or nipples that rest more than 2-3 centimeters lower than the average. This operation requires the nipples to be lifted and repositioned, and excess skin is removed. One major advantage of this surgical approach is that the outer and inner skin layers are repositioned to the middle, resulting in the patient’s breasts having a cone-like shape. The upper breasts and nipples are not flat and look naturally beautiful.
4. Anchor Breast Lift
This surgical approach is suited for patients with extreme sagging or nipples that rest more than 3 centimeters lower than the average. In this operation, surgeons will perform breast reduction surgery to reduce the size of the breasts and reposition the nipples to the appropriate position. Thus, this surgical approach requires horizontal incisions to achieve the natural and desired breast shapes and sizes.
With this surgical approach, breast tissues are lifted while also directly readjusting the breast structures. Areolas and nipples are naturally repositioned, resulting in a more youthful look. If patients’ areolas are too large, excess areola skins will be removed to achieve a more beautiful and desired result.
The four characteristics of beautiful breasts
beyond just size, are defined by the boundaries or “footprint” of the breasts. These boundaries are categorized into four sides:
Upper Boundary :
The upper boundary is defined by a line in red, which lies just below the uppermost part of the groove between the arms and breasts. Breasts positioned higher than this line may appear overly raised, while those below may seem lower.
Lower Boundary :
The lower boundary, often referred to as the IMF (inframammary fold), is marked by a green line and situated between the sixth ribcage bone. This is the location of the breast’s base. During breast augmentation surgery, the incision under the breast typically falls within this area.
Outer Boundary :
he outer boundary, represented by a yellow line, extends along the sides of the breast. It runs straight or slightly backward, generally no more than 1-2 centimeters from the outer edge. Some people prefer this as it provides a side boob appearance, but others may not like it due to the possibility of arm rubbing against the breast’s edge.
Inner Boundary :
The innermost boundary, indicated by a purple line, is typically located about 1-2 centimeters from the midline of the breastbone. The greater the distance, the wider the gap between the breasts, while minimal distance can lead to no cleavage. For a balanced appearance, it is advisable to have a gap of around 3 centimeters between the breasts.

A Half-Moon incision above the areola: For patients whose breasts are slightly sagging. Patients undergoing this method will gain the advantage of an incision scar that is hidden by the areola and is barely visible.

How much pain will I be in once the anesthesia wears off?
The patient will be in a little pain once the anesthesia wears off.
How permanent are the results from undergoing a Breast Lift?
Undergoing a Breast Lift may permanently fix the sagging of the breasts but does not ensure that it won’t sag once more due to factors such as age.
When will the incision scar fully heal?
The scar will be permanent but the skin and tissue will fully heal within 2 months after undergoing the surgery.
Will undergoing a Breast Lift diminish my ability to feel sensations around my areola?
This is dependent on what surgical technique the plastic surgeon decides to use but the patient can usually still expect to feel sensations around their areola as per normal after the surgery.
Will undergoing a Breast Lift decrease the size of my breasts?
It may make your breasts look slightly smaller as the plastic surgeon removes excess skin during the procedure. However, the patient can simultaneously undergo a Breast Augmentation to add volume.
Am I able to simultaneously undergo a Breast Lift and a Breast Augmentation?
Yes, the patient can simultaneously undergo a Breast Lift and a Breast Augmentation.
Once I undergo a Breast Lift, am I still able to breastfeed normally?
This is dependent on what surgical technique the plastic surgeon decides to use but 90% of patients are able to breastfeed as per normal. However, this may not be the case if the nipple and areola area need to be re-transplanted or repositioned.
